The Next Phase of the GLP-1 Boom Is About Choice, Access, and Follow-Up

The first wave of the GLP-1 era in the U.S. was defined by scarcity. Patients often waited for backlogged prescriptions, pharmacies rationed supply, and demand outpaced what manufacturers could deliver. That period is largely over. Tirzepatide and semaglutide are now regularly available, the range of clinical options has widened, and platforms such as Mochi Health offer structured care pathways that may include medications like compounded tirzepatide and compounded semaglutide where a licensed provider determines it clinically appropriate. The conversation has shifted toward what quality of care accompanies that access and whether it is thorough enough to produce lasting results.

From Scarcity to Selection

For much of 2023 and 2024, patients and clinicians navigated a constrained supply environment. Tirzepatide came off the FDA shortage list in December 2024, and semaglutide followed in February 2025. Supply stability has raised new questions about how providers, patients, and care platforms should approach a landscape where multiple treatment options are genuinely available at the same time.

That shift matters because semaglutide and tirzepatide are distinct products with different underlying mechanisms. Semaglutide, the active ingredient in Novo Nordisk’s Ozempic and Wegovy, works through the GLP-1 receptor. Tirzepatide, the active ingredient in Mounjaro and Zepbound from Eli Lilly, engages both the GLP-1 and GIP receptors. Clinical trial data for tirzepatide set new benchmarks in obesity medicine, and its performance has made receptor mechanisms a more active part of clinical conversations around prescribing.

Both medications carry similar side effect profiles. Nausea, constipation, and gastrointestinal discomfort are common, especially at the start of treatment and as doses increase. Providers typically use graduated titration schedules to manage tolerance over time.

What Choice Actually Requires

Selecting between active ingredients, formulations, and dose levels generally requires a thorough clinical evaluation of the individual patient. Prior treatment history, metabolic health profile, tolerance patterns, and coverage considerations may shape what a provider recommends. Identifying the right option for a specific patient takes more than a brief questionnaire.

Yale research on online prescribing found that patients could obtain GLP-1 prescriptions through some digital platforms with minimal clinician interaction. The depth of that first clinical conversation can set the foundation for everything that follows. A medication producing strong results for one patient profile may produce very different outcomes for another, and distinguishing between those profiles requires a real intake process with a qualified provider.

As access to GLP-1 treatments has broadened through telehealth, consistent clinical oversight at the point of prescribing has become a central issue in how care quality is measured. Expanding access and ensuring quality access are two separate things.

The Role of Follow-Up in Long-Term Weight Care

Obesity medicine now operates largely within a chronic-disease model, where treatment unfolds over years. Within that framework, follow-up care carries weight that a single prescription visit cannot address alone. Dose adjustments are common as a patient’s response changes over time. Side effect management, progress plateaus, and shifts in a patient’s broader health circumstances may all require ongoing clinical attention.

A patient who begins a GLP-1 treatment course might encounter moments where their medication needs adjustment, their side effect burden needs management, or their overall approach to weight care needs to evolve. Whether clinical access exists at those moments often determines what a patient does next.

The structural difference across care platforms comes down to what happens after a prescription is filled. Some models prioritize front-end access and offer limited support in the months that follow. Others build the ongoing care relationship into the service structure at the outset.

Mochi Health structures its model around continued access to a physician care team after the initial video consultation and prescription. That access can cover dose management, side-effect questions, and the clinical guidance that may arise long after the initial visit.

Compounding in the Stabilized Market

Compounding remains a clinical tool in the post-shortage environment, and its proper scope has narrowed. A licensed pharmacist at a state-licensed 503A pharmacy can prepare a compounded formulation for an individual patient whose clinical needs cannot be met by commercially available products. A provider may determine that a patient requires a dose or formulation that brand-name options do not offer.

Compounded medications are not approved or evaluated by the FDA for safety, effectiveness, or quality. The FDA has clarified that price alone does not justify compounding a medication that essentially copies a commercially available product. Compounding decisions may come from licensed providers who have assessed a specific patient’s documented clinical needs. The end of the shortage period has made that clinical basis a stricter requirement across the board.

What the Next Chapter Looks Like

The structural conditions that shaped the early GLP-1 boom have changed considerably. Supply has largely stabilized, clinical literature has deepened, and a wider range of care models has entered the market. What distinguishes platforms at this stage is what they build around the prescription. The patients who may be best suited to achieve sustained results are those whose care includes a genuine clinical relationship, consistent follow-up access, and a provider prepared to navigate a more complex treatment landscape than existed just two years ago.
